Daily Nurse Visit Program

At Chilliwack Pharmacy, we understand that managing medications can be difficult and overwhelming, especially for those with chronic illnesses. That's why we offer an innovative program to make it easier for you and your loved ones. Our nurse will come to your home on a daily basis to assist with the administration of medication, monitor vitals and report to the pharmacist and treating physician to make any necessary medication changes. We believe that this program will help to improve the quality of life for our patients and their families.

At this program our nurse will come to your home on a daily basis to:

 

-Assist you/ your loved ones with the administration of:

  • Oral medication

  • Insulin

  • Inhalers

  • Eye drops

  • Injections (Vit. B12, Biweekly or Monthly Psychiatric Injections)

  • Etc...

-Monitor :

  • Blood sugar

  • Blood pressure

  • Weight

  • Vital signs requested by physician

*All medication administration times, readings and vitals are monitored through Electronic MAR for each patient*

 

-Report to:

Pharmacist/Treating Physician on a Daily/Weekly basis to make regimen changes as directed.
